Top section has been cut away of front to show seaman's name (Pack, J.A.) and serial number (R 330054) handwritten and printed on first page. Black and white photograph of Pack on reverse of front cover. Biographical details at front along with discharge details handwritten and stamped throughout, dated 1945 to 1953. Loose documents at reverse.
