'Merchant Navy Personnel' Form. This form is to be used only when seamen who have been discharged are immediately re-engaged on Articles for further service on the same ship. It was issued to George Eric Rushworth of the JAMAICA PLANTER. Black print on white paper. Handwritten details, a carbon copy of original. Blue 'Mercantile Marine Office' date stamp for 10 Aug 1943.
