Typed in black ink on cream paper. Two pages attached with a paperclip. Report by the H.M. Inspector/Medical Inspector of Factories into a case of toxic jaundice, causing death to an individual working at the Royal Ordnance Factory, Glascoed. Dated 28th October, 1940.
