'Merchant Navy Personnel' Form. This form is to be used only when seamen who have been discharged are immediately re-engaged on Articles for further service in the same ship. It was issued to George Eric Rushworth of the M.V. SALACIA. Black print on both sides of pink paper. Handwritten details a carbon copy of original. Purple 'Mercantile Marine Office' date stamp for 26 May 1944.
