Blue wallet type cover with white print on front - Board of trade symbol on top, membership number (917175) and 'Continuous Certificate of Discharge' printed on lower half. Oblong cut out to show signature hand written on first page. Dated 1917 - 1930. First pages give biographical details on Samuel Richard Davies and two black and white photos of him at reverse. First half of booklet has been used.
